Updates on hemodiafiltration worldwide: practices, outcomes and mechanisms

Online hemodiafiltration (HDF) represents the most efficiency renal replacement therapy per time unit by combining diffusive and enhanced convective clearances.
Recent interventional and observational studies have shown that clinical benefits and outcomes are positively correlated to dialytic convective dose delivered. Interestingly, these results appear to be independent from the substitution modality used (i.e., post versus predilution) but still in a dose-dependent manner with a minimum threshold value. Implementation of best clinical practices are then crucial to ensure regular delivery of adequate dialytic convective dose. Clinical practices will be reviewed and integrated in a practical prescription and monitoring algorithm.
A brief summary of HDF outcomes will be provided in line with most recent evidences based on large cohort studies, interventional randomized controlled studies and further meta-analyses.
Finally, mechanisms supporting clinical benefits of HDF will be reviewed and categorized according to their level of evidence.
